I believe if you've earned achievements on Steam that are from profile restricted games (the ones that show "Steam is learning about this game" on the store pages), they won't count towards the achievement showcase on Steam. This site will still count them towards your overall totals though.
So that perhaps could explain the difference. I did try running a rescan of your activity and it's still showing 5423 total.

Oh, I see. That's very interesting. Is there any way I can see those games and the achievements in them on my Steam easily? I just find it so odd Steam says I have 436 games, but Exophase says I have 531.
Probably the best way is to add your profile to SteamHunters.
On the profile pages there, it will give a breakdown of how many achievements you've earned in restricted games. You can also filter by restricted games only. This profile for example: https://steamhunters.com/id/A_FAK/games
