These already seem to be listed under PC in the manual database.
Not sure if you were referring to the tracked Epic Games listings though? In any case they don't have achievements so won't be listed in that section.
This is normal, the site currently only tracks Ubisoft challenges which are separate from the achievements. I don't know if it'll be possible to track the achievements in the future, they aren't shown outside of the client PC app and only the logged in user can see them - can't view friends' achievements, etc.

Does the website add itch.io games to the database at all like Hexcraft: Eventide Sigil?
Yes, I've run a rescan on it. Achievement has been removed.
I've added that one under PC. We don't have any sort of automatic import for those though. The manual database is populated from GiantBomb currently. So unless they are listed there, we'll have to add them.

Here is the full list of all 44 Nuclear Throne achievements worth 1,000 gamerscore.
I think the developer intended it to be viewed as more of a full screen image when looking the original size https://m.exophase.com/xbox/awards/o/8714ggd.png But yeah, doesn't scale down that well unfortunately.
